Please see a doctor/nurse for Morning After Pills within 72 hours / Emergency coil up to 5th day
Pregnancy
If you think you are pregnant, please do a pregnancy test and confirm pregnancy and then book to see the Community Midwife.
If you are unsure as to continue with the pregnancy contact the Pregnancy Advisory Service for advice.
Sexual Health
If you have any concerns about sexual infections contraception or pregnancy, book to see Nurse or Dr Nair Any advice is completely confidential even for teenagers under the age of 16. Free condoms will be supplied. Information will not be divulged to parents without your consent. We will soon be supplying self test kits for Chlamydia.

Mens Health

Men over 40 years :
Recommended to have Blood Pressure check, BMI, You can do BP, height weight and BMI yourself at the surgery. Blood sugar and cholesterol can be checked at surgery / the local Pharmacy. You should have an assessment of your 'Cardiovascular Risk' ie calculate the risk of having a stroke or Heart attack if you smoke, have a family history of early heart attacks or if you are diabetic or have high cholesterol in the family.
Men Over 60 :
In addition to the above, prostate, sexual health can be discussed. See the doctor for a health check and discuss any health issues.

- Primary Care Mental Health Services. 775551 You can self refer to this service for Anxiety, Depression, Bulimia etc.
- Physiotherapy. 0845 113 0602 Monday to Friday between 9.00 am and 4.00 pm). This is a self book service, but has to be referred by the doctor/nurse
- Chiropody 77 8433/8430—Lichfield house 619955 This service is not available now for routine nail cutting etc. Referral via doctor/nurse. Ask for a referral at reception and state reason for referral.
- Citizens advice Beaureaux - If you need advice about housing problems, rejected benefits etc

Smoking Cessation
Smoking cessation clinic is run by Debbie Leng. Book to see her if you are motivated. We can prescribe patches and tablets as part of overall weight management plan.

Obesity & Weight Management
The weight management clinic is run by Laura Hawkins and Debbie Leng. They can advise you about diet and exercise, refer you to specialised exercise classes or dietician. We can also prescribe medicines as part of overall weight management plan.

Travel Prescriptions and Medicines

PATIENTS RIGHTS AND RESPONSIBILITIES
Patients Rights
- Be registered with a General Practitioner
- Change doctor if desired
- Be offered a health check on joining the Practice
- Receive emergency care
- Receive appropriate drugs and medicines
- Be referred to a specialist or second opinion if they and the GP agrees.
- Be treated courteously by all staff at the practice
- Be given sufficient information about their condition to make an informed choice about investigation and treatment.
- Be referred to the hospital/professional of their choice
- Have the right to review their medical records; subject to the relevant Acts.
- Confidentiality - You can withhold consent for your records to be seen by others. You can refuse to have your records uploaded on to the National Database
- Be treated without discrimination on race colour sex or sexuality
Patients responsibilities
- To keep booked appointments or cancel in good time
- To know that appointments are for one person only.
- Normal appointment duration is ten minutes
- Respecting other's rights by being courteous to all staff
- Letting us know when you change your address or telephone number
- Only requesting a home visit when it is truly necessary for medical reasons when a patient is too ill or immobile to come to surgery.
- Use the out of hours service responsibly i.e.: in a genuine emergency.
- Observing the no smoking and no mobile phone policy.
IT IS YOUR RESPONSIBILITY TO ATTEND APPOINTMENTS AND IF YOU MISS MORE THAN ONE APPOINMENT WE MAY ASK YOU TO MEET WITH THE MANAGER TO DISCUSS THE MATTER
ZERO TOLERANCE
The Practice will not tolerate any form of verbal or physical abuse towards the staff or clinicians. Any person doing so will be removed from the list with immediate effect
